Specifications include, but are not limited to: 1. Develop a community of practice plan to include health topics and social determinants topics to be covered; necessary trainings or skills needed to be a successful family navigator; and proposed timeline and schedule of convenings of community of practice. 2. Implement a Community of Practice cohort for Family Navigators by convening and facilitating family navigators virtually and/or in person to develop recommendations on necessary support needed for families. 3. Develop evaluation forms to measure effectiveness of family navigators and assess family concerns and needs, as well as those needs being met. 4. Assist with developing a Playbook for Family Navigators to implement a family navigator model in a community by addressing necessary resources, consent forms needed. 5. Assist with developing a training plan based on assessing trainings for Family Navigators and associated trainings for families in community hubs to address resources on screenings for individuals, family trainings, and referrals for social determinants of health. 6. Work with the DOH Project LAUNCH and its partners to assess the effectiveness of the family navigators in community hubs, assess community and family needs and impacts of navigators, and develop recommendations for trainings for navigators so that both the Awarded Contractor and DOH Project LAUNCH can make modifications as needed throughout this contract period.
