Specifications include, but are not limited to: provide OT /PT (consult and direct) and evaluations to students with disabilities who ARDC deems appropriate. The District seeks qualified and licensed OT /PT service providers to implement children’s OT/PT individualized educational plans, conduct evaluations/eligibility per District and Federal Educational guidelines, and attend meetings with multi-disciplinary teams to discuss such services. OT /PT providers must follow District standards for assessment/evaluation/eligibility and if eligible, develop individualized educational plans for those students eligible for services. OT /PT provider must comply with timeline for services per federal law. Completed evaluations and documentation must be submitted prior to billing. All selected proposals will be contingent upon available funds.
